To request a prescription refill call 480.912.5959, or submit an online request using the form below.

Please enter the prescription number(s) you wish to refill at this time. This number is located on your prescription label.


OUT OF REFILLS?

If you are out of refills, we will automatically contact your provider for you. Please allow extra time to complete your refill as many providers are unable to respond to refill requests in the same day. If you notice you are out of refills, it is helpful to contact us a few days earlier than normal, so you do not run out of your medication.
